Launching May 2024! Prior to enrolling or completing registration forms, you must attend an information session. You can conveniently self-schedule this service. Please note: you cannot be currently enrolled in our meal replacement program to participate in this pilot program and must be able to attend weekly online classes Wednesdays, 4-5:30 p.m. You will receive a link to join your information session 15 minutes before the session to your text, email, and kp.org appointment center.
After your orientation
After you have attended an information session, you must complete the following medical questionnaire. Then you’ll complete the following steps to get started. We cannot accept forms until you have attended orientation.
