Bariatric Required Weight Checks

In order to provide you with the best medical advice, your bariatric physician and surgeon need accurate, up to date weights in your medical record at the following times:

  • Within 4 weeks prior to your initial telephone appointment with the bariatrician (this is required or your appointment will be rescheduled)
  • At Workshop #5 (this is optional but highly recommended)
  • Within 1 week of completing Workshop 10 (required) 

Delays in getting your weight checks will delay your surgery scheduling process.

Any weight noted in your chart from an in-person Kaiser Permanente appointment meets this requirement. If you have not had a recent in-person weight documented, follow the steps below to complete one. 

You can complete a weight check at a Positive Choice Clinic or at a Target Clinic. 

  POSITIVE CHOICE Weight Check-in: 

  • Visit one of the following Positive Choice clinics and let the Service Representative know you are there for a bariatric weight check. 
  • No appointment necessary, but please only come in on the following days/times:
  • Garfield (6th floor) Monday to Thursday 10am-12pm and 2-4pm
  • San Marcos (Building 4) Tuesday 10am-12pm and 2-4pm
  • Otay Mesa (Building 4) Wednesday 10am-12pm and 2-4pm
  • Bostonia Thursday 10am-12pm and 2:30-4pm

TARGET CLINIC Weight Check-in:

  • Schedule a visit at www.kptargetclinic.org
  •  Select the “blood pressure check with LVN” option– tell them it’s a weight check when you arrive for your appointment
  • Fill out name (as stated on medical card), birthdate, legal sex, phone, email when prompted
  •  In “help us prepare for your arrival” box, add “weight check for bariatric surgery” with your workshop number (1, 5, or 10)
