We offer a comprehensive program to help you achieve your short- and long-term goals from the convenience of your home.

Using meal replacements will help you lose weight and manage many health conditions. Our focus is to offer a program that considers your personal goals when it comes to health and well-being. While focusing on your safety and success on the program, we provide the tools for you to focus on maintaining your results. Using at home scales and blood pressure cuffs and home delivery service, our remote program option reduces your need for weekly clinic visits!

Program Benefits:

  • 75% of people with type 2 diabetes achieve normal fasting blood
    glucose off of medications
  • HbA1c drops an average of 2 points
  • Insulin sensitivity and glucose tolerance increases
  • Liver enzymes normalize
  • Fat in the liver and pancreas can decrease
  • Blood pressure decreases (⅓ of participants are able to discontinue medications, ⅓ of participants are able to greatly reduce medication)
  • Total blood cholesterol drops an average of 81 points and triglycerides drop 55.6%
  • After 8 weeks, the average weight loss is 15% from starting weight and 5 years after weight loss, ⅔ or more people maintain half or more of the weight they lost off.

Your journey happens in phases

This is a medically-supervised meal replacement program. While you’ll meet with the medical team weekly, you can reach out with questions between visits.

1. Five Daily Meal Replacements Phase

While on meal replacements, you will attend weekly clinic visits at a Positive Choice location on your class day. During these visits, you will pay your weekly program fee, select and pick-up your meal replacement order, and check in with our medical team. You’ll also attend weekly classes and complete bi-weekly lab draws. These are a requirement while on this medically-supervised program.

2. Three Daily Meal Replacements Phase

As a next step, meal replacement program participants complete a course designed to help you develop a lifelong wellness plan. This includes the nutrition, fitness, and healthy habits you need for long-term success.

3. Ongoing Support Phase

Our robust Booster program includes classes, workshops and support groups. These provide ongoing learning and community – important ingredients for staying interested, focused and excited about taking good care of your health.

As part of your program, be sure to complete these tasks:

  • Order your meal replacements every two weeks before the end of the day on Monday (if you don’t, your order will arrive late). You are still charged your full weekly fee, even if you don’t place an order.
  • Send us your weekly data by 12:00 p.m. on your class day.
  • If you are required, upload your blood glucose and/or blood pressure data weekly by 12:00 p.m. on your class day.
  • Visit your nearest KP lab for lab draws every two weeks.

Remote Program Frequently Asked Questions

Participation Requirements
  • You must be a KP member in Southern California. While we currently offer the program to San Diego County members only, we will be expanding to select clinics in the LA area in 2026.
  • You must have a BMI >25 and meet other medical criteria. Please attend an information session to learn more.
  • At this time, you are required to come to a San Diego area KP clinic every 2 weeks in order to participate. 

Remote Program Advantages

In our remote program:

  • You do not meet with our medical team or pick up meal replacements in our clinics each week. Instead, you will have virtual appointments, upload your weight and other data weekly, and order your meal replacements for home delivery. However, you will visit your local KP lab for blood work every 2 weeks, on your own schedule.
  • You receive a scale and blood pressure cuff in lieu of our in-clinic SECA body composition testing.
  • While you still pay weekly, the remote program weekly fee includes your shipping charges, Maintenance (for our full- and modified-programs) and Booster program fees. There is no separate charge for these services.
  • You will pay shipping fees for your meal replacement orders, however total program costs between programs are otherwise the same for most participants.
Weight and Blood Pressure Checks

For your safety, to participate in this program, you are required to provide weekly blood pressure readings and weight recordings on the mornings of your weekly classes.

To complete this, you can either:
1. Visit a nurse clinic or Target clinic weekly. See instructions below.
2. Purchase and use a home blood pressure cuff and scale and report it through our website positivechoice.org/remote

Examples of reliable home BP monitors:
•  Omron (series 3, bronze, silver, gold, or platinum) 
•  Microlife upper arm digital blood pressure monitor 
•  Welch Allyn Home 1500 series or 1700 series 
•  A&D UA-767F blood pressure monitor

If you already have your own blood pressure machine, bring it to an appointment or nurse clinic visit to confirm accuracy.  Ask staff to check your blood pressure with your device and with the office device to compare readings and validate your machine. You can also compare your home scale weight with one at our clinic. Slight differences (within a few pounds) in scale weights between home and clinic scales are expected.

How to Complete a Drop-In Nurse Clinic Visit
• Visit your nearby KP clinic and request a nurse clinic visit at the check in desk. See below for locations and hours.
• You may experience a wait as these are walk-in visits
• Let the staff know you need a documented weight and blood pressure check

How to Schedule a Target Clinic Visit
• Schedule a visit at www.kptargetclinic.org
•  Select the “blood pressure check with LVN” option– tell them it’s a weight check when you arrive for your appointment
• Fill out name (as stated on medical card), birthdate, legal sex, phone, email when prompted
•  In “help us prepare for your arrival” box, add “weight and blood pressure check for Positive Choice”

How many readings should be taken?
For accurate reporting, take 3 sets of morning readings for best results.  Each set should include 2 readings.

What are the best times to measure blood pressure?
Early morning.  Measure within 1 hour of waking and before taking any morning medicines.

Do’s and Don’ts:
• Use the bathroom before you measure your blood pressure so that you are comfortable.  
• Do not cross your legs.
• Sit in a chair with your back supported and both feet resting on the ground.  
• Do not position the cuff over clothing.
• Flex your arm at the elbow and rest it on a flat tabletop or other surface for support .  
• Do not talk when you take your reading.
• Put the cuff in direct contact with the skin of your arm.
• Do not smoke, have caffeinated drinks, or exercise within 30 minutes before taking your blood pressure.
• Sit in a quiet room.

Log In Tips

You will have 3 account log ins during this program. Write down your log in information in your Program Guide: 

1. KP.org: this is your Kaiser Permanente, online medical record that is not specific to this program. We will communicate with you through the secure messaging system and you can view your medical letters, appointment information, lab results, and more through your account.

2. E-store: this is where you will log in to place your meal replacement orders. Orders are placed every other week by the end of the day Monday. You can view the ordering calendar above. If you forget your password, do not try more than 2 attempts or you may get locked out. Instead, reset your password in the My Account section. If you are having trouble placing an order, watch the video above. 

3. My Healthy Journey App (available through June 1, 2026): this is an app by Coachcare that you will download from your device’s App Store to a mobile device. Your Bluetooth scale and blood pressure cuff will connect to this app. Each week you must upload your data to our medical team. Upload data by noon on your class days. If you use a password saver service, it will appear as Coachcare.

Order Information

Ordering:

You will need to place an order for meal replacements every two weeks while on the program. You should select 8 boxes of meal replacements per order as part of your regular program fees. Additional items are available for a charge. Orders must be placed by the end of the day on Mondays of your order week. Late orders may not be processed on time and can lead to gaps in your meal replacement program. You will be charged your full weekly program fee, even if you forget to place an order. We recommend using the calendar above to set reminders on your phone.

Tracking orders:

When you place your first order, you will set up an e-store account and log in. This allows the system to save your shipping information and for you to track orders. Once you place an order, you will receive a series of emails as well, they will be from 820direct, this is the shipping warehouse. To track your order, log in to the e-store and view your order history.

Order issues:

Bluetooth Devices and App

You will receive a tape measure for monitoring your waist circumference, a Bluetooth body composition scale, and a Bluetooth blood pressure cuff. These devices can be synced with many smartphones.

Each week, before noon on your class day, you must send your weight and waist circumference. You may also be required to send your blood pressure readings (view your program prescription letter to verify if you are required to do this). Weekly data updates are a requirement for remaining in the program. 

Medical Supervision

Prior to starting the program, you complete a thorough medical consultation that includes a detailed medical and personal history questionnaire, lab work, and EKG, and a medical clearance visit.

Once enrolled, you must complete weekly data sharing as required by your program prescription. This includes weight and waist circumference and may also include blood pressure and blood sugar monitoring.

We will schedule a follow up visit to occur within a few weeks of starting your program and strongly encourage consistent video or phone appointments with our medical team. You can self-schedule above or call to book these visits. You are free to contact the medical team or schedule appointments as needed but we recommend at least once a month check ins on your progress. Contact information is listed above.

Support with nutrition or transitioning off meal replacements

Need help with your meal plan? Or are you transitioning of your meal replacement program? Attend a weekly support session that meets on Wednesdays from 5:30-6:00 p.m.  Click the link below to log in to the TEAMS session.

Once you are no longer using meal replacements, you will enroll in the Power of Lifestyle, Solutions, or BLAST modules for specific support in continuing or maintaining your efforts. Booster sessions are open for long term support as well.

Lifelong Booster Support

 What to expect:

  • Over a dozen monthly classes
  • Variety of rotating topics
  • Support groups
  • Order meal replacements as needed

After completing meal replacements, you are invited to attend unlimited Booster classes for ongoing support. There are a variety of class topics, workshops and support groups offered every month. Take a look at the monthly calendar online and register for as many sessions that interest you… at no further cost!

 

As long as you are in the Maintenance class or attending monthly Booster classes, you are welcome to purchase up to 8 boxes of meal replacements per month. You can buy these at a Positive Choice clinic location or on our website and have them shipped to you.
